Hospice RN's on our Long Term Care, Home Care, Admissions, In-Patient Facility and After Hours Teams
Utilize a systematic process to assess the physical and emotional needs of the patient to provide the framework from which patient care is planned.
Recognize, report and document significant findings/changes in the patient's condition to the clinical manager and/or physician.
Assess the need for medical supplies, equipment and medication on a regular and consistent basis.
Assess the need for alternative levels of care options when appropriate.
Contribute to the development of an Interdisciplinary Treatment Plan in collaboration with the multi-disciplinary team providing a comprehensive plan of care.
Identify additional problems/needs and contributes to the initial plan of care started on admission.
Identify patient/family educational needs and develop a teaching plan utilizing resources available.
Participate in ITM. (team meetings)
Implement and direct plan of care in an organized and efficient manner in accordance with applicable policies and procedures.
Implement physician directed care in a timely manner.
Implement nursing interventions and procedures for physical and emotional needs based on the plan of care.
Plan the administration of and/or administer medication with knowledge of actions, contra-indications, side effects and desired outcomes.
Communicate with other team members and/or outside services and nursing agencies to provide coordination of care.
Provide information and direction to the HHAs/CNAs, documenting supervision according to policy.
Complete electronic documentation accurately, concisely and according to policy.
Report all infections/suspected infections on the appropriate forms
Attend/prepare the body after death, providing emotional support to the family and direction for arrangements as needed within established guidelines.
Continually evaluate the plan of care, the nursing care provided and the patient's response to the care so that adjustments may be made accordingly.
Evaluate the effectiveness of the nursing interventions in meeting the identified physical and emotional needs, making revisions on the plan of care.
Communicate with the physician informing him/her of the patient’s status on a daily basis for Inpatients and in writing every thirty (30) days for Home Care patients.
Notify the physician at the time of death.
Perform monthly chart reviews as assigned.
Perform other related work as needed and as directed by the clinical manager and/or director.
Is available for holiday, on-call, weekend, illness and vacation coverage as needed.
Current FL RN license required
Valid driver's license required
Minimum of 2 years nursing experience required
Long Term Care or Home Care experience a plus!
BSN and Hospice experience preferred
Bilingual English/Spanish a plus!
